Carfentanil is a synthetic opioid that is similar to, but 1,000 times more powerful than, fentanyl. Carfentanil is known to be used by veterinarians to sedate elephants and other large mammals. The drug is also 10,000 times more powerful than morphine.
Carfentanil can be difficult to identify, as it is clear, odorless and highly soluble in water. The unremarkable nature of carfentanil makes it convenient for drug dealers to cut it into other drugs, in order to increase the base drug’s volume and provide a potentially stronger high. However, the symptoms that come with being exposed to carfentanil are, on the other hand, easy to identify and can appear within minutes. These symptoms include clammy skin, respiratory difficulties, disorientation, pinpoint pupils and drowsiness.
Carfentanil is a lipophilic chemical, and can easily cross the blood-brain barrier. It has very rapid onset of effects, but is also shorter acting than fentanyl.
For pain relief, a unit of carfentanil is 100 times as potent as the same amount of fentanyl, 5,000 times as potent as a unit of heroin and 10,000 times as potent as a unit of morphine. This is despite only having 14-135 times higher affinity for the μ receptor. Despite its higher potency, carfentanil is less fatal than fentanyl in rats.
The effects of carfentanil, including overdose, can be reversed by naloxone. Carfentanil is legally controlled in most jurisdictions, but has veterinary uses for anaesthetising large animals.
